If you call or send your CREDIT CARD NUMBER, please note that we will need the following information:

                                        1. Name as it appears on the credit card   

                                        2. Credit Card Number   

                                        3. Expiration Date 
                                       
                                        4. Billing Address for the Credit Card               

                                        5. Shipping Address (if different from Billing Address)                

                                        6. Phone number           

                                        7. The V-Code (a 3 digit code that is printed on the back of the credit card,
                                        following the account number which is generally found along the signature strip)

            For MAIL IN, remember to add shipping and handling charges of $3.00 for the first item, 50¢ each additional item.

            For International mail orders (U. S. dollars only) shipping is $7.00 for the first item, $1.00 for each add'l item.

Texas residents add 7.25% sales tax.
